Client Consent Form

To ensure your safety and provide the best care, all first-time clients are required to complete a Client Consent Form before attending their treatment. This form allows you to disclose any relevant medical conditions, allergies, or medications that may affect your procedure.

Returning clients do not need to refill the form unless it has been more than 6 months since their last treatment or if there have been any changes to their medical history.

Aesthetic Treatments – Cancellation and Rescheduling Policy

At Bellucci Aesthetics, we value your time and commitment. Our policy ensures fair and efficient appointment management for all clients.

Deposit Policy

A non-refundable deposit of £100 is required to secure your appointment for all aesthetic treatments excluding botulinum toxin. This deposit is non-refundable if you cancel or fail to attend your appointment for any reason.

If you attend your appointment, the £100 deposit will be deducted from your total treatment cost on the day of your visit.

Cancellation Policy

• Cancellation within 72 hours: If you cancel your appointment less than 72 hours before your scheduled time, your £100 deposit will be forfeited in full.
• Cancellation within 24 hours: If you cancel your appointment within 24 hours of your scheduled time, your deposit will be forfeited in full.
• No-Show Policy: Failure to attend your appointment will result in the full forfeiture of your deposit. This applies to all aesthetic treatments.

Attendance on the Day

When you attend your appointment, your £100 deposit will be deducted from the total treatment cost on the day. Please arrive on time so all necessary consultation and consent steps can be completed before your procedure.

Medical History Disclosure

Prior to paying your deposit, you must disclose any medical conditions or medications that may affect your treatment. This ensures your safety and optimal results. Failure to provide accurate information may lead to rescheduling or cancellation with deposit forfeiture.

Commitment to Treatment Plans

If you book a treatment package or series, you are committing to the full number of sessions specified at the time of booking. Once confirmed, the number of treatments cannot be altered or reduced. Attempting to do so will result in deposit forfeiture, and you will remain responsible for the full package fee.

Professional Standards

All treatments at Bellucci Aesthetics are performed by a qualified aesthetics injector. If you have any concerns during or after your treatment, please contact us immediately so we can assist you promptly.

Late Arrivals & Appointment Policy

Please arrive on time. If you are more than 15 minutes late, your appointment may need to be rescheduled or cancelled at the discretion of your provider, and your deposit may be forfeited.

Child Policy

Clients who bring children must be accompanied by another adult to supervise them during treatment. No child under 16 may be left unattended in the clinic. Failure to comply will result in cancellation and deposit forfeiture.

Age Restrictions

For injectable and skin treatments, clients must be 18 years or older. We strictly adhere to GMC guidance and reserve the right to refuse treatment for clients under the age of consent.

Medical Disclaimer

All clients will receive full information about their procedure, including benefits, risks, and aftercare. Common injection reactions such as redness, swelling, or bruising are expected and temporary. It is the client’s responsibility to raise any concerns with the practitioner prior to treatment.

Bellucci Aesthetics cannot be held responsible for adverse effects resulting from medically appropriate treatments where risks have been explained in advance.

Aftercare

Your injector will provide tailored aftercare guidance following treatment. Please follow all instructions carefully to maintain results. Individual outcomes may vary based on anatomy and product choice.

Payment and Data Privacy

All payments are processed securely through our third-party provider, Tide. Bellucci Aesthetics does not store personal payment information. All client data and medical forms are handled in accordance with the UK Data Protection Act.

Agreement

By paying your £100 deposit, you confirm that you have read, understood, and agreed to the terms of this policy, including cancellation and rescheduling rules. You also acknowledge the importance of completing the Client Consent Form before your appointment.
